Hello! I am new to this group and just starting the protocol to induce lactation. I have started birth control and am waiting for my domperidone to come in. When I ordered it, the pharmacy was adament about the max dose being 30 mg a day…..my lactation consultant said not to worry but my doctor who is very supportive was a little concerned and wanted to keep me on the 30 mg dose to see how it went. I have no biological kids and no fertility issues, but therefore have never nursed. I have only talked with people who have previously nursed, who have tried adoptive breastfeeding. Has anyone who has not previously nursed been successful? You read a lot about what to take when but does anyone have some practical advice on what this looks like on a day to day basis? I have been blessed to find peope supportive but not experienced….any thoughts are appreciated!!

First, congratulations on starting this journey!  I have breastfed my two adopted children and have been taking domperidone for more than 2 years almost continuously, with a small break (a few months) between kids.  I have also breastfed 2 natural kids, so I can’t attest to the success if I had never lactated, but my results were very good.  I did have to supplement, but only a couple of ounces at first, more as they got older and hungrier.  I supplement with donor breastmilk (see milkshare.com, or facebook groups Eats on Feets and Human Milk for Human Babies if you are interested in this.)

I did research on domperidone before taking it and came to the conclusion that it is safe in doses up to 120 mg daily.  I offer this information as a friend only, although incidentally I am a physician, so I trust my own research.  I have taken 20 mg 4 times daily and for a short time took it 6 times a day, which gave me some perimenopausal symptoms - dryness, low libido.  At 4 times a day with frequent nursing, I have not gotten my period since before the second baby was born 9 months ago, a definite plus in my book!

I also until recently took blessed thistle and fenugreek, 3 capsules each 3 times a day.  I stopped because they were giving me heartburn.  My day looked like this:

Breakfast:  6 capsules, 2 domperidone
Lunch:  6 caps, 2 domperidone
Dinner:  6 caps, 2 domperidone
Bedtime:  2 domperidone

It was a lot of pills but it really worked.  When I stopped the domperidone when my first baby was 11 months old, my milk production dropped like a stone and he nearly quit nursing on his own.  However, when I resumed it a few months later for baby two, my milk came in within a day (!) and he suddenly couldn’t get enough.  At 2 years old now, his favorite sentence is “more booby, mama!”

I second zambarano!  The max dose listed on the Newman-Goldfarb protocol page is 80 mg daily (4x20mg).  I was on that for a year with my first adoptive nursling and am on it now with my second.  Near the end of the first year, I tried increasing my dose to 120 mg/day to see if I could stop supplementing because the Lact-Aid cleaning duties were causing my wife some burnout.  I had liver function checked at the end of that and everything was absolutely perfect.  I ended up dropping back to 80 mg daily because my daughter was so used to the Lact-Aid that she would not latch without it and breaking her of the habit was more trouble than it was worth!  She self-weaned at 10.5 months.

I only had a month to do the Newman-Goldfarb protocol before my daughter was born. I had never given birth or nursed before (no fertility issues). I have taken the 80 mg daily (4x20mg) the entire time -  I am still nursing my 2 year old. I never produced much milk at all. Maybe half an ounce per feeding (not sure as I could never get anything from a pump but that’s how much she left in the SNS once my milk supply came in). I supplemented with donor milk and formula. Between 18 month and two years my daughter sometimes wants the SNS but mainly just nurses with me. The older she gets, the less about the milk it is for her and the more it is about being a nursing toddler needing attachment and security. So I am glad that I stuck with it and didn’t get discouraged about the amount of milk.

No issues with domperidone for me either. But I cut back at one point and my supply dwindled. So it’s important to know that it works to build AND maintain the supply.

(BTW- I got my dom from a pharmacy at first for $.50 a pill. Now I order it online for $.10 a pill. I use inhousepharmacy.biz and have never had a problem.)

The Lact-Aid cleaning/filling is a chore, but so are bottles if you do that. The important thing is to have at least 6 of them so you just have to do the routine twice a day.
